Here’s the form you’ll need to obtain reimbursement for invoices relating to the project for which UFB has provided support.

Please note: Before completing the form, please bring the following scanned PDF documents as requested in the e-mail you received:

  1. Bank details for your NPO’s account (less than 2 years old)
  2. The 2 forms (“acceptance of conditions” and “SPF Finances”) duly completed and signed
  3. Invoices (in the name of the asbl)
